Clinical Provider Agreement
Our providers strive to provide the best care coordination for our patients and work collaboratively to ensure that the best patient healthcare outcomes are achieved. All providers will engage in care coordination for each individual patient. Patients of Isaac Medical Group will be seen by the first available provider on their appointment day and time unless specifically communicated otherwise. Isaac Medical Group providers include: Physicians, Nurse Practitioners, and Physician Extenders. Patients can request specific providers based upon preferences and will be advised of a specific day and time to schedule their office visits.
Membership Agreement
The terms of this membership are effective upon enrollment. The membership may be terminated upon written notice by either party. This membership expires after one year / 12 month term and will automatically renew unless otherwise notified by the member.
Members will be charged $100.00 for early termination of this membership. To cancel membership with Isaac Medical Group without penalty: Members must contact customer support by writing within 30 days of opening membership. Members must contact customer support 30 days before the end of their one year contract. Termination of this membership by Isaac Medical Group will be made if members account lapses 60 days or more due to non-payment, membership will then be suspended and sent to collections. To reinstate the membership the entire balance of missed payments including late fees must be paid in full.
Appointment Policies and Procedures Agreement
Members are expected to call the office to schedule your appointment or book an appointment online. Members are expected to arrive on time for all appointments. Members are expected to call if running late for an appointment at least 15 minutes before the visit.
Members are expected to call to cancel an appointment - a 24-hour notice in advance is required.
Members, who miss 3 or more appointments, are more than 15min late for 3 or more visits, or any combination will be subject to a $50 Appointment Policy Violation fee which will be assessed to the members account.
Pharmacy and Laboratory Agreement
Medication and labs are priced separately and are NOT included in the monthly membership fee.
All Dispensed Medications and Lab Draws are to be paid in full at the time of each office visit. Medication and Lab work charges can either be drafted from your linked account on file or paid in cash.
Automatic Credit/Debit Card Billing Authorization Agreement
Members enjoy the convenience of automated billing via ACH with account information Credit/Debit Card transaction. All requested information is required to complete the monthly billing profile. Upon approval, you will have the option to setup a monthly / quarterly or annual auto- deduction. Payments are made directly through our secure portal. Failed monthly membership charges for ANY reason are subject to additional fees. Failed payment notices are sent to the member via email and the pending charges persist. Members who do not wish to use our automated billing systems can pay for monthly membership fees cash and are REQUIRED to pay for no less than 3 months upfront.
